The state(s) where tryouts will be held. If you will hold tryouts in multiple states then select multiple states here.

The primary contact person you want people to reach with questions etc.

The email address of your primary contact person.

Phone

The phone number of your primary contact person.

Club Address

Geographic location of the club for display on map. Ideally a street address like "9 Main Street, Sometown, FL" but could just be a zip code or town and state if you don't have a specific physical address for the club.
